Poland Launches the Free Medical Check-Up Programme Moje Zdrowie

Poland has replaced the "Profilaktyka 40+" programme with a comprehensive preventive health programme called "Moje Zdrowie" (My Health). It is open to all adults aged 20 and over. You can sign up through your patient account (Internetowe Konto Pacjenta – IKP), the Moje IKP app, or directly at a primary care clinic (POZ).

Prevention – Starting Young

The aim of the programme is to encourage people in Poland to undergo preventive check-ups regularly and to take care of their health. Minister of Health Izabela Leszczyna notes: "We are used to monitoring the health of children and young people — giving them regular check-ups, tracking their development. Yet very often we, as adults, forget to take care of ourselves in the same way. That is why we are introducing a health balance sheet for adults as well, to build a healthy habit — looking after your own wellbeing."

The programme will help identify health risks and problems, including cardiovascular, metabolic and oncological conditions, and address them in a timely manner.

Discussing Results with Medical Staff

All patients complete questionnaires — online via IKP or on-site at their POZ clinic. People aged 60 and over are asked to answer three additional questions aimed at detecting signs of dementia. The completed questionnaire is sent to the chosen clinic (POZ), which will issue a referral for tests. The clinic has 30 days to contact the patient. The scope of tests depends on the questionnaire responses. After the check-up, the patient meets with a doctor or nurse to discuss the results.

A Personalised Health Plan for Everyone

Basic test package (for all participants): → full blood count (morphology), → glucose, → creatinine (with eGFR), → lipid panel (total cholesterol, LDL, HDL, triglycerides), → TSH (thyroid hormone), → urinalysis.

Extended package (depending on age and questionnaire): → ALT (alanine aminotransferase), → AST (aspartate aminotransferase), → GGT (gamma-glutamyl transferase), → PSA (men), → anti-HCV (hepatitis C), → lipoprotein (a) — performed once within the programme.

"Moje Zdrowie" Replacing "Profilaktyka 40+"

The "Moje Zdrowie" programme replaces the pilot project "Profilaktyka 40+", which was previously a one-time initiative. The new programme has significant advantages: → covers a wider group of patients (from age 20), → includes questionnaires and a follow-up meeting with medical staff, → will be available at all POZ clinics across the country.

Based on the results, each patient will receive an individualised health plan.

Who Can Participate in the Moje Zdrowie Programme and How Often?

→ People aged 20–49once every 5 years → People aged 50 and overonce every 3 years

Patients who completed the questionnaire under the "Profilaktyka 40+" programme before the end of April 2025 may still use it until the end of May 2025. Participation in "Moje Zdrowie" is available 12 months after the last check-up under the "Profilaktyka 40+" programme.

Ukrainians registered in Poland's healthcare system with access to primary care (POZ) are also eligible to take part in the Moje Zdrowie programme.
