The free programme of preventive medical examinations and laboratory tests for citizens aged 40 and over will remain available until the end of April 2025. The initiative aims to promote regular health check-ups, which help detect diseases early and prevent their progression. Ukrainian citizens enrolled in the public health insurance scheme or holding a PESEL UKR number are also entitled to a free medical examination under the Profilaktyka 40+ programme.

Below we look in detail at how the programme works.

The Polish Ministry of Health announced the extension of the free diagnostic programme "Profilaktyka 40 plus" until 30 April 2025. The initiative, aimed at people over 40, has already reached more than 5 million participants, according to data from the National Health Fund. The programme offers comprehensive examinations that allow various conditions to be detected at an early stage, including hypertension, cancers, and other medical problems. Participants may undergo examinations more than once, provided they observe a minimum interval of 12 months between visits. It is important to note that the programme is available not only to Polish citizens, but also to Ukrainians who hold public health insurance or a PESEL number with UKR status. The website of the Polish Ministry of Health underlines the importance of the programme in the context of limited access to healthcare during the pandemic. The Ministry stresses the need to make up for missed preventive check-ups during that period, since early detection of diseases significantly improves the chances of effective treatment. Citizens are encouraged to take advantage of this opportunity to look after their health.

What tests are available under the Profilaktyka 40+ programme?

The diagnostic package for people of both sexes includes:

  • full blood count with differential and platelets;
  • total cholesterol or lipid profile;
  • blood glucose;
  • liver function tests: ALT, AST, GGTP;
  • blood creatinine level;
  • urinalysis;
  • blood uric acid level;
  • faecal occult blood – immunochemical method (iFOBT).

Men are additionally tested for prostate-specific antigen (PSA). Regardless of sex, all programme participants have their blood pressure, weight, height, and waist circumference measured, and their body mass index calculated.

How to take part in the Profilaktyka 40+ programme?

Log in to your patient account via this link. Find and click the "profilaktyka" button at the top of the screen. Select "ankieta" (questionnaire) and answer the questions. The questionnaire can be completed at a medical facility (if it cooperates with the National Health Fund) or by calling 989. Contact details and a list of medical facilities are available on the government website. People under 64 must answer questions about:

  • weight, height, and waist circumference;
  • cardiovascular disease risk;
  • physical activity;
  • diet and alcohol consumption;
  • smoking and other forms of tobacco and nicotine use;
  • cancer risk.

Answers should be selected from a list or by ticking the correct options. If you are unsure how to answer a question, you may skip it. However, answering as many questions as possible will allow a better assessment of risk factors and help identify the most appropriate tests.

People aged 65 and over must answer questions with only "yes" or "no", covering:

  • blood pressure, cholesterol, and glucose/blood sugar tests in the past year
  • an active lifestyle
  • smoking
  • treatment for thyroid or endocrine system conditions
  • gastrointestinal symptoms
  • excess weight in the patient or their relatives
  • gastrointestinal cancer in the patient or their relatives

After completing the questionnaire, you will receive an electronic referral for tests. You then need to book an appointment – log in to the Central e-Registration platform, select your referral, and choose a medical facility. During the visit, you will need to bring your passport and PESEL UKR number (for Ukrainian citizens). How often can you use the diagnostic service? If you have already had free tests under the Profilaktyka 40+ programme, you can repeat them only after one year.
