How to Join the Electronic Queue at the Ukrainian Consulate in Poland?

What Services Are Available at Ukrainian Consulates in Poland?
At Ukrainian consulates in Poland you can:
→ apply for a Ukrainian citizen's foreign passport;
→ extend the validity of your passport;

Registering in the Electronic Queue at the Consular Office
To register, follow these steps:
- Fill out the form at the link, specifying in particular:
→ the consulate you wish to visit;
→ the service you need;
→ your preferred date and time of appointment;
→ your personal details.

If you need several services, each one must be registered separately, choosing a different time slot!
Enter your real details when filling out the form and double-check their accuracy. Inaccurate personal information is grounds for refusal of appointment.
If you plan to come with family members — they must also be registered.
For one service, you should register only once, for one date and time.
- The application must be signed — with an electronic signature via "Diia.Pidpys" or BankID of the National Bank of Ukraine.
This helps the institution prevent queue-jumping, which used to be a common problem.
To sign electronically via "Diia", scan the QR code displayed on screen after completing the registration.
It is important that the smartphone or laptop you are using to fill in the form has a camera — you will be redirected to it and asked to perform various actions: blink, smile, turn your head, etc. You will also need to remember your "Diia" password — you will enter it during verification.
If everything goes smoothly, a message will be sent to the email address you provided during registration — you will need to confirm your consulate appointment. The consulate will then send a confirmation letter and a letter with the details and conditions of your visit. We recommend following all the guidelines mentioned in the letters. For example, you may be asked to arrive 10 minutes before your appointment.

The consulate may reschedule or cancel your appointment. You will be notified in advance — a message will be sent to your email.
If you cannot attend your appointment — you must cancel or reschedule it. To do this, find the consulate's letter in your inbox. The letter contains reschedule and cancel functions — choose one of them.
