Free Healthcare in Poland: When NFZ Coverage Ends and How to Avoid Medical Debt

After losing official employment or ceasing to pay social security contributions, a patient may lose access to state-funded treatment. According to the Pacjent.gov.pl portal, once the established deadlines expire, all costs for doctor visits or hospitalization must be covered by the patient.

When health insurance coverage ends

The general rule states that the right to NFZ services remains valid for 30 days after the last insurance contribution was paid. This countdown begins in the event of:

  • termination of employment (resignation or dismissal);
  • suspension or closure of a sole proprietorship (business activity);
  • an employee taking an extended period of unpaid leave.

You can verify your active health insurance status in the eWUŚ electronic system. The database is updated daily based on information from social insurance institutions (ZUS and KRUS).

Exceptions for students and graduates

For certain categories of individuals, the period for free healthcare access is longer:

6 months – for graduates of high schools, technical schools, and vocational schools (or after being expelled);

4 months – for university graduates (or after being expelled);

90 days – for individuals whose social pension payments have been suspended.

How to avoid paying for treatment without active insurance

If a patient receives medical care after their insurance has expired, there is a legal option to avoid paying the bills by applying for retroactive insurance. The relevant application must be submitted:

  • within 30 days of receiving the medical service;
  • or within 30 days of receiving an official letter from the NFZ demanding reimbursement for treatment costs.

Once the application is successfully processed, the obligation to pay the medical expenses personally is waived.

  • After leaving a job, you have exactly 30 days to visit doctors for free or complete ongoing treatment. Remember that family members registered under your insurance will also lose coverage after this period.
  • When changing jobs, ensure your new employer registers you with ZUS on time to avoid dangerous "gaps" in your medical coverage.
  • Regularly check your status in the eWUŚ system (e.g., via the Patient's Online Account – IKP) to ensure your right to free healthcare.
  • If an emergency occurs and you receive aid without active insurance, do not ignore letters from the NFZ. You have 30 days to apply for retroactive insurance and legally cancel the medical debt.
